Advance Care Directives Project

At some point in your life, there may come a time when you are unable to make a decision. If this happens, how would you want decisions to be made for you about your health care, living arrangements and other personal matters?

The District Council of Yankalilla, in partnership with the City of Victor Harbor and Alexandrina Council, is working together with the Fleurieu Advance Care Directives (ACD) Reference Group to develop a community service that will equip people with the information they need to complete their own Advance Care Directive.

We're looking for dedicated volunteers interested in undertaking training to support community members to complete their own Advance Care Directive.

If you are interested in finding out more about becoming a volunteer call the Community Team on 8558 0200 or email: council@yankalilla.sa.gov.au

Advance Care Directive Kits and Forms

Residents can purchase Advance Care Directive kits and forms from the Council Office at 1 Charles Street, Yankalilla. The kits cost $5 and additional application forms can be purchased for $1.
